Home Delivered Meals

Providing meals that meet at least one-third of the dietary reference intakes to individuals in their private homes
For waiver participants who cannot prepare or obtain nutritionally adequate meals, ensuring the provision of such meals to promote independence and ensure health, welfare, and safety

Non-Medical Transportation Services

Enabling individuals to gain access to community activities and resources specified by the plan of care/service plan
